Identification of potential candidates for Catastrophic Care Management and Disease Management is the obvious initial step of care management. Toward that end, BMS CareConnex™ has developed and utilizes a proprietary software program called AutoTrigger™. AutoTrigger™ filters all claims data every night to identify the most likely candidates for both Catastrophic Care Management and Disease Management. Once a candidate is identified, the AutoTrigger™ program builds a patient profile database and electronically refers the member to one of our nurses for further assessment and evaluation.

BMS CareConnex™ is modular and flexible. While all modules are designed to work best as an integrated suite, we recognize that each employer and plan administrator has unique needs and objectives based upon their demographics, claim utilization trends, and overall philosophy regarding managed care. Each module is available on a stand-alone basis. Discounted pricing is available for those clients requiring multiple modules. Further, within most of the service modules, we accommodate most requests to customize the product to suit specific client needs with little or no addition charge.
